As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Jeffrey Swartz as follows: • Your treatment and recordkeeping in this matter did not meet the expected standards of the dental profession in Ontario. • You failed to demonstrate basic clinical knowledge and understanding of endodontic procedure. By initiating an endodontic procedure without a pre-operative radiograph, you failed to meet the expected standard of the profession. • You failed to demonstrate any respect for your obligations to maintain complete and accurate patient records. The panel was particularly disappointed that you added information to your patient records after your receipt of the complaint from the College.
