As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Barinderpal Chagger as follows:

•	Dr. Chagger must be mindful of the consequences of poor implant case selection on patient harm and safety. In particular, planning for implant treatment without adequate bone, insufficient vertical height, and a poorly positioned nerve, increases the risks of implant failure, compromises periodontal health of adjacent teeth, increases the risk of permanent nerve damage and exposes the patient to unnecessary pain and suffering.

•	Dr. Chagger is cautioned to be mindful of recognizing the limitations of treatment given his level of expertise and experience and identifying implant cases that may be outside his scope of practice, in particular when complicating factors are evident prior to the commencement of treatment. In such circumstances, Dr. Chagger should refer to or consult with a specialist so as not to unduly put his patients at risk.  

•	Obtaining informed consent requires a discussion of the results of the use of diagnostic aides, such as CT scans, and the implications those results have on the success of treatment. When complicating factors are present that increase the risks of treatment, the patient must be well advised of the impact those factors will have on the treatment outcome. A fulsome discussion of this nature must be documented in the patient record. Dr. Chagger is also cautioned to ensure that lessons learned from previous remedial outcomes on informed consent, including courses and cautions are put into practice.
