As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Pulec as follows: • Dr. Pulec must obtain adequate informed consent from his patients, in particular when undertaking a unique or complicated course of treatment where the risks of failure are greater. Patients must be informed of and consent to all aspects of their treatment, and caution must be taken to communicate with the patient about the details of the procedure, in particular when complications arise. This discussion and the patient’s consent must be documented in the patient record. • Dr. Pulec inadequately case managed the restorative phase of implant treatment for his patient. He exercised poor professional judgment in altering an existing crown restoration over an implant that did not properly fit, which resulted in the crown continuing to fail. Dr. Pulec did not provide his patients with the option of referral to a specialist when treatment complications arose. • Dr. Pulec is cautioned for his failure to abide by the College’s Guideline on Implant Dentistry (2002) in place at the time of the treatment he provided to his patient, and in particular, his failure to conduct an adequate case work-up, diagnosis, and treatment plan for his patient, including the restorative treatment phase.
