Complaints & Reports Outcomes

Case File: 150028

Decision Date:
December 01, 2015

Caution

As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Hussein as follows:

     • He must attend to every patient with appropriate care, and he must ensure that he has allocated adequate time to provide proper treatment that meets the standard of care. Patients should not have to have treatment redone because he did not allocate enough time to complete the treatment properly the first time.

     • He should be conscientious of the time required to treat rampant caries, which takes longer than treating discrete caries.

     • He should be conscientious to use operating room time, which is limited, appropriately and responsibly, including allocating adequate time for the planned treatment.

Decision Date:
April 19, 2017

Caution

As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Hussein as follows:

• Dr. Hussein’s progress notes must accurately reflect, in detail, what occurred during each appointment. If template, or pre-written, text, are used in the progress notes, it must be customized for each patient and each appointment. While this is important for all parts of the progress notes, it is especially important with respect to informed consent discussions, which must always reflect the risks and benefits particular to the patient in question.

• Dr. Hussein must prioritize patients’ interest in his dental practice. In practice, this may include recommending against certain options, such as implants for patients whose medical and dental histories indicate the treatment is unlikely to be successful. Alternatively, it may include referring patients to specialists for consultation or treatment in complex cases.
